2. Print application

Review and sign the BAPP Physician Participation Agreement on page 6. Return only the signed page 6 with your BAPP Membership Application and enclose your payment for the first year's dues and the one-time credentialing fee. (Annual dues is $850.00 and the credentialing fee is $150.00.)
