Note:
Patients are encouraged to complete the Patient Medication Pocket Card. These pocket cards serve several purposes. The cards provide patients with a tool for maintaining a current list of medications, it educates patients to know the medications they are taking, a patient can record allergy information, it documents immunization history, and it meets the Joint Commission on Accreditation of Hospital Organizations (JCAHO) standards regarding medication reconciliation. Patients are encouraged to update the information as needed and carry the card at all times and present the card any time medical care is received.
